A wounded man in a Sydney hospital may be a gunman involved in a car yard shooting that left one man dead, police say.

About 2.45pm (AEDT) on Friday paramedics were called to MacArthur Auto Centre in Blaxland Road, Campbelltown, in Sydney's south-west following reports of two men being shot.

On arrival, a male thought to be aged in his 30s was confirmed dead as the result of a gunshot wound, an Ambulance spokeswoman said.

The second man, believed to be aged 24, was taken to Campbelltown Hospital by a man now being questioned by police.

The injured man was later transferred to Liverpool Hospital, where he was in a stable but serious condition after sustaining a wound to his neck.

A police spokesman told AAP he believed the gunman responsible "might be the one in hospital".

"I haven't been told of someone at large," he said.

"So it's probably the guy with the wound.

"It's believed the two people hit are the ones involved in the shooting," he said.

"So the deceased and the wounded is my understanding of it."

He added he believed the man being questioned by Campbelltown Police, who drove the injured man to hospital, was a witness.

A worker at a Tyrepower store next to the MacArthur Auto Centre said that after hearing two shots fired, he watched a man of Pacific Islander appearance flee from the scene.

"I heard the two shots, they were about 15 feet (five metres) from where I was working at the time," he told Fairfax Radio Network.

"I turned around and a guy has come out of the shop next door ... a big Islander fella ... and he sort of looked at the guy who worked there and goes "it wasn't me" and runs off, jumps in his car and goes off in a blaze.

"(Then) the owner comes out and says "Call an ambulance I'm shot, I'm bleeding.

"Then we find out five minutes later that he's off to hospital, shot ... and there is another guy in there dead."

The worker said at first he thought the two shots were fireworks, and the whole thing was a joke.

"I thought it was fireworks - people mucking around," he said.

"Even when the guy came out ... and he said `it wasn't me', I'm thinking they're having a bit of a joke.

"Then the other guy runs out with no shirt on, blood streaming down his chest saying, `I've been shot, call an ambulance'."

A crime scene has been established and inquiries into the incident are continuing.

Police say the incident is not related to Monday night's brawl at Rosemeadow.

Crime Investigator Dean Johnstone, from Campbelltown police, said more people needed to come forward to assist police with their inquiries.
